Geriatric Evaluation Clinic
Most older people live healthy and active lives. But, with advancing years, some individuals encounter chronic illnesses or disabilities. Monroe Community Hospital's Geriatric Evaluation Program was formed to help address the long-term health concerns faced by older individuals and their families. This consultative program was one of the first of its kind in the nation, and has gained national recognition since its inception in 1985.

The program is dedicated to helping older people maintain good health and independence throughout the aging process. A team of health care professionals, including a physician, registered nurse, and social worker, thoroughly assess the health and functional ability of each individual patient by conducting a series of examinations and visits. This comprehensive process allows the team to fully understand the heath care needs of each person in order to prepare a customized care plan for each individual.
