The admission process to a nursing home for long term care or rehabilitation can be difficult to navigate alone. Following are some commonly asked questions that may help you in this process. What is involved in the admissions process at MCH?

The admitting process for Monroe Community Hospital has two specific areas of review:

The Clinical Review process is performed by a Registered Nurse who evaluates and determines whether the health care professionals at MCH could meet the applicant's care needs. The information reviewed is based on standardized assessments, the medical record, and, for some complex cases, an on-site interview and record review.

The Financial Review process is completed by a skilled Financial Coordinator, who evaluates the types of insurances and payer sources for individuals being considered for Long Term Care or Rehabilitation. This process will consist of interviews with the patient or family, the insurance representatives, the referring facilities, and the MCH Business Office.

What information is needed to apply for long-term care services at Monroe Community Hospital?

If you are applying to Monroe Community Hospital and are currently living at home (either with or without Homecare Services), our Admissions Office will need the following information: A recent PRI, an up-to-date Medical Summary, and a Medication List.

If an individual is applying to Monroe Community Hospital and is currently being cared for at a hospital or another nursing home or chronic care facility, our Admissions Office will need the following information: Progress Notes, Physical Therapy and Occupational Therapy Notes, Recent Lab Tests, and H&P.

Do you need additional information to apply for short-stay Rehabilitation services at Monroe Community Hospital?

No. The information that we need to process your application is the same for both short-stay Rehabilitation and Long-term Care.

What is a PRI?

PRI is an acronym that stands for Patient Review Instrument. The PRI is a standardized assessment required by New York State prior to an admission to a nursing home in this state. The PRI form must be administered by a Registered Nurse (RN) or other qualified individual licensed to perform this assessment.
