OUR PROCESS
CLINICAL REVIEW

Our clinical review process is performed by both our Admitting Coordinator and/or Clinical Admissions Coordinator.  We evaluate referrals based on clinical needs, psychosocial needs and level of care.  MCH is a skilled nursing facility, meaning that we must determine if you or your loved one meet a skilled level of care for admission.  This is done by reviewing information on standardized assessments, medical record and for some complex cases, on-site interviews.  We work with our clinical teams here at MCH to determine if we can meet the need of the patient.  

FINANCIAL REVIEW

Because MCH provides a variety of skilled nursing services,

including sub-acute rehabilitation and 24-hour skilled nursing, we require a financial application for every referral.  Our financial review process is completed by our Financial Coordinator together with both our Admitting Coordinator and/or Clinical Admissions Coordinator.  We evaluate each referral's insurance and payer source prior to admissions. We review each case for financial approval individually and often requires discussion with the referral source and/or family, insurance representatives, referring faculties, attorneys and the MCH Business Office.

Once both the clinical and financial reviews are completed, our team will make the final determination for admission.  Bed offers will be made based on current availability at the time of the referral review.​

We work with all of the major health systems in Monroe County and surrounding areas.  Please contact your unit social worker to initiate a referral to MCH.

  • PRI/Screen within 30-90 days

  • Most recent History & Physical (H&P) from community primary care physician

  • Most recent immunizations

  • MOLST form

  • Health Care Proxy, Power of Attorney, Living Will, Guardianship  as applicable

Frequently Asked Questions

What is a PRI & Screen?

 

A PRI stands for Patient Review Instrument.  The PRI is a standardized assessment required by New York State prior to admission to a skilled nursing facility.  The PRI is completed by a Registered Nurse (RN) or other qualified individual licensed to conduct this assessment.  The PRI is used as part of our admissions process to evaluate the needs of the referral and determine the level of care.  If a referral is generated from a hospital setting, the hospital will complete the PRI. If the referral is coming from home or other community settings, there are agencies that can assist you in completing this.  Please contact us if you have further questions or need information on how to obtain a PRI.

What is the bed availability at MCH?

 

MCH has 566 beds overall, these include both sub-acute rehabilitation and 24 hour skilled nursing.  MCH offers a variety of specialty units for specific care needs such as respiratory needs and dementia care.  Bed availability changes on a daily basis and offers are made based on current availability at the time of the referral.

How much does it cost?

 

We work very hard to provide our services in the most cost-effective manner.  Like most other healthcare providers in the nation, we continue to face increases in healthcare costs.  Despite these challenges, we remain dedicated to providing exceptional health care services to those we care for. Please call to inquire about our current daily rates.

 

How long does the admissions process take?

 

This varies depending on the volume of referrals and bed availability.  We work with all the major health systems in Monroe County and surrounding areas to ensure our admission process is efficient. Referrals from home or other community settings may take longer to ensure we have all the information to perform a comprehensive review. We strive to provide timely communication. However, if there is a delay, you may call us to inquire about a specific referral or if you have additional questions.

 

Are there some referrals that you cannot accept?

 

Our admissions department receives on average 300-400 referrals per month.  We review each one based on the criteria listed above.  There may be clinical or financial barriers to admission at MCH.  We strive to eliminate barriers prior to admission, but in some cases, we may not be able to meet the needs of every individual who is referred.  If you have questions regarding a referral, please contact us at 585-760-6022 and, we would be happy to speak with you. You can also schedule a tour by clicking below.
